I understand that once I complete my child’s enrollment in Kinder Spanish and that if I decide to cancel enrollment, I must notify Cristina Restrepo the Director of Kinder Spanish by email cristina@kinderspanish.com 30 days in advance.

 

 

I hereby grant permission for my child to attend Kinder Spanish and to be under the Kinder Spanish supervision during the tutoring program at the preschool. I understand that the Director Cristina Restrepo of Kinder Spanish will not be held responsible for personal injury occurring as a result of Kinder Spanish participation. I also understand that insurance will not be provided by Kinder Spanish, and I agree to provide my own. Therefore, I will be responsible for any medical or other charges in connection with your child’s attendance of Kinder Spanish. I hereby release Kinder Spanish, its agents and employees from liability for injury, loss or death of my child that may occur while participating in Kinder Spanish. 

 

I do hereby consent and agree that Kinder Spanish has the right to take photographs/video of my child and to use on the company website (www.kinderspanish.com) and promotional material without compensation. Your child’s name will not be revealed. If you prefer we do not photograph/video your child please notify us by email at (cristina@kinderspanish.com)
